BLACK Membership Passport 2020-21

The BLACK membership passport form is a Professional membership. You are a USFS Coach, Judge, Technical Specialist, Accountant, Announcer, Music Player over the age of 18 as of July 1, 2020. You must have an official appointment from United States Figure Skating to be considered for this membership. Coaches must actively teach at a CSC facility - SSIA or FSC to be eligible for the CSC professional membership.

  • CSC Volunteer Policy

    Colorado Skating Club Volunteer Policy – updated 2014 Volunteer support is an essential part of our club, as our volunteers run the clubs activities, business and events. Club dues primarily cover our US Figure Skating member fees and give us a small financial base on which to operate. Without the help and support of our members through volunteer hours contributed, we cannot perform our financial functions. In order to spread the volunteer hours among members, Colorado Skating Club requires a minimum of 10 volunteer hours per membership year for each skater. This requirement is subject to change in future years, will be set each year prior to the start of each skating year, and will be stated on the membership form. During CSC’s membership process or renewal process, a volunteer policy must be signed annually by each skater (or skater’s representative, i.e. parent). With the implementation of this policy, we hope to be able to support our skaters more than ever and with time, expand the clubs activities. Collegiate members are not required to volunteer, however we encourage participation if the student is living in the metro area. Below are the responsibilities of members to fulfill the volunteer hours: 1. Each skating member must sign this volunteer policy when renewing membership. 2. Each skater/volunteer is responsible for a minimum of 10 volunteer hours for CSC sponsored events. 3. Each skater/volunteer is responsible for filling out the on-line volunteer hours form. 4. Hours need to be recorded monthly using the on-line form. 5. Each skater/volunteer is responsible for checking in with designated volunteer coordinator at the CSC event (failure to check in will result in hours not being recorded). 6. Volunteer hours may be performed by member’s family and/or skater. 7. Hours will be tracked in 15 minute increments and verified by the volunteer coordinator. 8. All volunteer hours for skating year must be fulfilled by June 30th of the membership year. 9. If you sign up for hours but do not work them, they will not count. Below are the responsibilities of CSC Board/Committee Members: 1. CSC Board will provide ample opportunity to fulfill volunteer hours at events such as but not limited to: Funtastics (Mile High event in April), Cup of Colorado/Colorado Gold (CSC Competition in August), CSC Test Sessions (every other month), CSC Annual Banquet (Feb.), Showtime on Ice (April or May), CSC Committee work (throughout the year). 2. CSC Volunteer Coordinator will post opportunities on CSC Website and will be sent in the CSC e-mail blasts. 3. CSC Volunteer Coordinator will make every attempt to schedule the hours/times for which you have requested. 4. CSC has two volunteer awards given out at the annual banquet: Doug Goss Volunteer award for a senior member or parent of skater and the Garlene Braack Jr. Volunteer award for a junior member. CSC Board determines the winners each year. 5. An individual tracking form will be made available to members for use in tracking hours (this can be printed). We need all members to help to make our club run successfully. Should you not fulfill your hours or know ahead of time that you are not able to volunteer, we ask that you make a donation to the Colorado Skating Club so that we can pay for caterers or other services the club may need to hire during Cup of Colorado and other events. The recommended donation if you cannot fulfill your 10 hours is $200. If you can only fulfill approximately half of your hours, the recommended donation is $100.   • Waiver/Release/Assumption of Risk

    Colorado Skating Club WAIVER/RELEASE/ASSUMPTION OF RISK AGREEMENT In consideration of me being permitted to participate in any way in the US Figure Skating (USFS) and/or Colorado Skating Club (CSC) sponsored Activities (“Activity”), I agree: 1. I understand the nature of Colorado Skating Club activities and believe I am qualified to participate in such Activity. I further acknowledge that I am aware the activity will be conducted in facilities open to the public during the Activity. I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the Activity. 2. I FULLY UNDERSTAND that: (a) USFS and/or Colorado Skating Club Activities involve risks and dangers of SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS AND DEATH (“Risks”); (b) these Risks and dangers may be caused by my own actions, or inaction!s, the actions or inaction!s of others participating in the Activity, the condition in which the Activity takes place, or THE NEGLIGENCE OF THE “RELEASEES” NAMED BELOW; (c) there may be other risks and social and economic losses either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES incurred as a result of my Participation in the Activity. 3. I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS USFS and/or Colorado Skating Club, their respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and if applicable, owners and lessors of premises on which the Activity takes place (each considered one of the “Releasees” herein) from all liability, claims, demands, losses or damages on my account caused or alleged to be caused in whole or in part by the negligence of the “Releasees” or otherwise, including negligent rescue operations and further agree that if, despite this release, I, or anyone on my behalf makes a claim against any of the Releasees named above, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS LIABILITY, DAMAGE OR COSTS ANY MAY INCUR AS THE RESULT OF ANY SUCH CLAIM.

  • CSC Print and Electronic Media Waiver

    Please answer these questions as they pertain to the CSC Print and Electronic Media Waiver. The Colorado Skating Club (CSC) publishes several print and electronic media for use by its membership. CSC may take pictures of or video of and write articles about its members, skating club activities including but not limited to competitions, ice shows and the annual banquet. These are for use on the CSC bulletin board at South Suburban Ice Arena/Family Sports Center, CSC website and as part of any CSC publications including but not limited to CSC newsletter. Member's information may only be included in these publications with the written consent of the member or parent/guardian (if the member is a minor).

  • CSC Code of Conduct/SafeSport Statement

    Colorado Skating Club is committed to creating a safe and positive environment for members’ physical, emotional and social development and ensuring that it promotes an environment free of misconduct. The following code of conduct applies to all participants in activities hosted, supported, sponsored or engaged in by U.S. Figure Skating and Colorado Skating Club including but not limited to competitions, exhibitions, training camps and local rink activities. I recognize that my participation or my child's in all the activities associated with U.S. Figure Skating and Colorado Skating Club is an honor and privilege that carries certain responsibilities. I agree to fully abide by the rules and guidelines set forth by U.S. Figure Skating or its properly designated agents. U.S. Figure Skating Member Code of Conduct GR 1.02 I recognize that my participation in all activities hosted, supported, sponsored or engaged in by U.S. Figure Skating, including but not limited to competitions, exhibitions and training camps, is an honor and privilege that carries certain responsibilities. I agree to fully abide by the rules and guidelines set forth by U.S. Figure Skating or its properly designated agents. As a precondition to participation in activities hosted, supported, sponsored or engaged in by U.S. Figure Skating, I will adhere to the following tenets in good faith: A. I will exhibit the highest standards of fairness, ethical behavior and genuine good sportsmanship in all of my relations with others. B. I will not damage public or private property. I understand that I may be held financially responsible for damage deemed to be wantonly or willfully executed on my part, and that I may be subject to disciplinary action by U.S. Figure Skating. C. I will not use or possess illegal drugs, and I will not engage in criminal activity. I understand that, if I am found to use or possess illegal drugs, or if I am found to engage in any criminal activity during any activity hosted, supported, sponsored or engaged in by U.S. Figure Skating, I may be subject to criminal penalties as well as penalties imposed by U.S. Figure Skating. D. I will adhere to the rules of U.S. Figure Skating and the host organization at all activities hosted, supported, sponsored or engaged in by U.S. Figure Skating. E. I will comply with all applicable anti-doping rules including, but not limited to, ISU and USADA anti-doping rules. F. I will conduct myself in a manner not detrimental to the welfare of figure skating. I understand that my actions reflect on U.S. Figure Skating and the sport of figure skating both positively and negatively. I understand that if my acts, statements, or conduct are considered detrimental to the welfare of figure skating by the appropriate authority, I may be subject to penalties imposed by U.S. Figure Skating pursuant to GR 1.04. G. I understand that the penalties that may be imposed may include, but are not limited to, loss of future international selections, loss of financial support from U.S. Figure Skating and its Memorial Fund, and loss of participation in activities hosted, supported, sponsored or engaged in by U.S. Figure Skating. H. I understand that all disciplinary proceedings will be conducted pursuant to Article XXV, Section 3, of the U.S. Figure Skating bylaws, and that my rights and remedies are derived there from.
